Integrated Event Investigation

The occurrence of an unexpected, significant event has immediate as well as long-term impacts on an organization.

Such events might include:

• A serious injury
• A forced outage of a power plant
• An unexpected plant trip
• A critical equipment failure with significant financial impact
• A project failure – budget overrun, schedule slips, etc.
• A structure failure
• An electrical system blackout

These and similar events signal breakdowns at many levels – human performance, equipment reliability, organizations and programs, and/or management failures. To assess the event’s cause, and more importantly, to prevent the recurrence of similar events, breakdowns at all levels need to be investigated.
