The purpose of a common cause analysis is to find the common causes for seemingly diverse types of low level problems identified by field observations or condition reports. Unless the common cause is identified and eliminated, these seemingly diverse types of problems could not be solved. This course teaches students how to perform common cause analysis periodically to identify common causes for:
Human performance problems
O&P problems
Executive management problems

In a business, there are many low level events. Each of it may not deserve a full-blown root cause analysis. However, when they are analyzed together, root causes at the higher level of business, such as deficiencies in organizations, programs, executive management system, or even executive’s decision system could be identified and eliminated. Unless these common causes are identified and eliminated, low level events could not be totally eliminated.
