The Human Performance Inspector module is used to improve Individual performance, Pre-Job Briefs, Safety Management Readiness and Organizational Readiness in power plants and manufacturing companies to identify and mitigate error traps before work begins. Users are able to engage more attentively and know to-do and not-to-do items before carrying out tasks. Quality and accountability are the core to Human Performance Inspections. With KALE™, the result of the Human Performance Inspection is a report output which contains a readiness level that can be saved, downloaded, printed, or shared with other users.
Individuals can use HPI to predict whether errors are likely before the start of a task. This readiness meter consists of two parts. The first part is dealing with a self-assessment of personal readiness in Capabilities, Attitude, Personality, and potential Error-Free® mental states (also known as the CAPESM matrix) for the task about to be performed. The second part is to assess the existence of the high-risk situations in the (TAPESM matrix). A display of the Error-Free® readiness meter with a scale from 0% to 100% tells us how prepared we are to be Error-Free® for a task. Once error-likely areas are identified for an individual, compensatory actions can be applied to mitigate error possibilities.
Once a task is complete, individuals can go back and document near misses or errors for reflection and lessons learned. Over a period of time (monthly) or over a number of the tasks (say 10), the results are printed for the user to see common patterns of problems. With this data, the user can improve further by being able to more accurately identify problematic areas in CAPESM and TAPESM matrix and formulate better mitigation measures.
The Pre-Job Brief human performance inspector module can ensure a high quality PJB by inspecting 4 aspects: human readiness, operating experience, plans/process/procedure readiness, and site readiness. After going through the checklist, you can rely on the readiness score to anticipate how well the job will be performed.
By performing an HPI Pre-Job Briefing (HOPSSM) we are able to be become situationally aware according to the specific job. Generally tasks, procedures, roles, and responsibilities are addressed in a meeting prior to a job beginning.
This module ensures the PJB thoroughly covers the readiness of the involved workforce, plan preparation, lessons learned from similar jobs, single point vulnerabilities, and site readiness. Often, PJB’s are performed as nothing more than a formality, and organizations rarely audit their readiness. The results of the HPI-PJB lists out topics that have not been addressed in the PJB or by the Organization, so that corrective actions can be identified and taken.
Error-Free®’s experts determine that the quality of the Pre-Job Briefs could be quantified by two independent parameters:
Safety professionals can easily utilize the HPI readiness meter to track the current safety management system and prevent injuries from line-of-fire, overexertion, slip/trip/falls and equipment related, etc. By reviewing Human Readiness, Onsite Hazards Elimination Readiness, Procedure Readiness, and Situation Awareness Readiness, injury rates can be efficiently reduced. Furthermore, with the Safety Management Readiness Meter, specific injury rates can be calculated and predicted based on system status.
Organization – A strong organization that can provide high quality has elements of adequate personnel, high expectations of behavior standards, layers of protections in place as well as sufficient flow of knowledge and information. When it comes to Organizational Human Performance Inspections the target areas are: LOP management, Error-Free® behavior, flow of information & knowledge, and team inspection.
To reduce the non-quality costs, PII’s research has found that the inspection strategy should move up to the human performance level, which is a critical input to all products or jobs.
In the past five years, PII developed the integrated strategy of Human Performance Inspection. With a routine checkup, Human Performance Inspection can determine whether individuals, Pre-Job Meetings, Safety Management Systems, and organizations are in great condition or if there are weak areas that need improvement in order to provide quality work. All inspection results are tracked and monitored for performance trending.
