The solution is the Error-Free 3-Layers-of-protection (LOP) and 1-Improvement Model. This model is developed by PII over years using big data analysis and practical experience to identify how different events occur from various errors, and to identify the barriers that can detect and stop these errors from generating an event.
This strategy is developed as a breakthrough after ISO13485. Previously, ISO13485 and risk management did not consider the probability of error. However, from the 3-LOP and 1-Improvement Model, human error induced organizational and program risks can be quantified and managed.
Human errors can be eliminated through Error-Free habits and preventive measures such as improving existing programs and procedures, as well as filling the gaps of missing elements for organizations through root cause analysis, and performance monitoring and trending programs.
