SPV Error Meter

The SPV Error Meter module has been used to improve Pre-Job Briefs in power plants and manufacturing organizations so they can identify high risk situations that need to be addressed before starting a job. PII’s research in the reduction of error has found that one of the most effective measures of risk reduction is to use the SPV Error Meter before work starts which will predict the risk probability.

In order to be Error-Free®, a risk analysis should be done to determine the error propensity of a job. The risk level provided by using this tool will give the ability to perform an adequate PJB and risk mitigation for the specific job.

According to Error-Free® training, there are 4 categories of high risk situations: Time, Activity, People, and Environment

This module diagnoses existing error traps and provide mitigation techniques for each category.

If needed, it is then possible to mitigate the contributing factors. The SPV Error Meter quantifies a risk level of each high risk situation ahead of time so personnel can be aware and mitigate them if possible using the suggested actions. The risk level provided by using this tool will give the ability to perform an adequate PJB for the specific job. According to the risk assessed, organizations will also be able to designate the proper level of PJB to ensure a successful job is carried out. In order to be Error-Free®, a risk analysis should be done to determine the generalized error probability of a job. According to the Error-Free® trainings, there are 4 categories of high risk situations: Time, Activity, People, and Environment. This module will diagnose error traps and provide mitigation techniques for each category. The purpose of the Error-Free® Technology is to enable you to create an Error-Free Zone® (or culture) within which all preventable errors are cost-effectively prevented. The mitigation action for each contributing factor is listed in the report. It is important to consider all Single Point Vulnerabilities and Point-of-No- Returns. The result of the SPV Error Meter analysis is a report output which contains a risk level that can be saved on KALE™, downloaded, printed, or shared with other users.
